Project Description

The MCH (Maternal and Child Health) GOT Transitions Project, funded by the Arizona Department of Health Services,  is a training program that offers Maintenance of Certification (MOC) Part 4 credits upon completion. This project is designed as a comprehensive training program aimed at equipping healthcare practices with the essential guidance required to implement effective transition policies. By focusing on increasing the percentage of youth receiving crucial transition services, enhancing the proportion of practices with established transition policies, and raising awareness about the necessity of transition planning—particularly for individuals with special healthcare needs—the MCH GOT Transitions Project is poised to make a profound impact on the quality of care and support provided to our community's youth.  We are currently enrolling practitioners, if you would like more information, please contact aridlab@arizona.edu.
