Arizona Health Care Transition Performance Improvement Program
The Arizona Health Care Transition Performance Improvement Program is a comprehensive training program designed to equip healthcare practices with the necessary tools and guidance to develop and implement effective transition policies and workflows. It is funded by the Arizona Department of Health Services and offers an opportunity for healthcare professionals to earn Maintenance of Certification (MOC) Part 4 credits. Our mission is to improve the percentage of youth receiving vital transition services, increase the number of practices with established transition protocols, and raise awareness about the importance of transition planning, especially for individuals with special healthcare needs.
Through the training program, we aim to make a meaningful difference in the quality of care and support provided to youth and young adults in our community. We are currently enrolling practitioners into the program! Email aridlab@arizona.edu to enroll.
